• Legal and DBA (Doing Business As) name

• Home address, and Business address

• Primary contact name

• Principal telephone number

• Principal fax number (if applicable)

• E-mail address

• Internet site address

 Business Information

• Type of ownership (Sole Proprietor, Partnership, Limited Partnership, Non-Profit)

• Social Security number

• If incorporated

• State of incorporation

• Federal tax ID

• The date your business operation began

• A complete description of the products and/or services you sell

 Sales Data & Business Information

• Credit card refund and submission policies

• Delivery or product fulfillment information

 Banking Information for Billing Purposes

• Bank Name

• Routing Number (the 9-digit number at the bottom left of your check)

• DDA or Account Number (the checking account number that begins after the Routing number)

 Payment Type Selection

• Determine the types of online payments you want to accept from the following selection:

• Electronic Checks (eCheck)

• Discover®

• MasterCard®

• VISA®
